I'm doing my AOS paperwork, and Woking clinic gave me just a "Vaccination Documentation Medical Worksheet Two", looks like a photocopy, and nowhere does it say "DS-3025".

The results also state "Vaccine history incomplete" "Applicant may be eleigible for blanket waiver....", and I'm confused as to if I need to fill out an I-693. Any suggestions????

I will be so glad when I can look back and laugh about this whole insane process!!

Thats the correct sheet.

Did you have all the shots you need or did you have to get them in the USA?

If you have all your shots and they are all current you don't need a 693. You only need to make a copy of the vaccination sheet and include it in the package.

Usually the incomplete is because some shots are not medically or age appropriate and there for you get an automatic waiver for that.

Your interview took 67 days from your I-129F NOA1 date.

AOS was approved in 2 months and 8 days without interview.

ROC was approved in 3 months and 2 days without interview.

That is the correct form even though it doesn't say DS-3025 on it. The original was in the sealed envelope you got from the medical that you gave to the consulate. The photocopy that you received is perfectly fine.

Almost everyone gets the blanket waiver box checked because many vaccines e.g. rotavirus are either for babies or 65+ year olds and thus are inappropriate for you.
