Hello

Okay so I am in the process of changing my marriage name on my Canadian documents, currently my Passport with my K1 visa is still in my maiden name. I got my 10 year Green Card finally it was approved back in march after being denied last May due to some issues caused by a family member on my part. I been back in canada since Feb due to illness which I got permission to travel back for medical treatment, which is when in the process my green card was also approved after a full investigation and stokes interview. What is the process like for getting your name changed on passport? Do i apply directly to the counsalte here? or do I apply for a new passport as Passport Canada states to change yoi name you need to fill out a whole new application?

You apply for a new passport with supporting documentation: http://www.ppt.gc.ca/cdn/section1.aspx?lang=eng

I was confused by one thing in your post though. You are not entitled to medical treatment in Canada when you are not a resident of Canada.

Anyone is entitle to treatment, I think you meant not entitled to FREE treatment right?

I am Canadian, I still have health coverage, I live in both Canada and US, coverage goes based on how long you are physically in the country, and that your a citizen, as long as your present for 153 days in a 12 month year you are still eligable.

Sorry I must have been typing at the same time as you above.

It seems you are walking a fine line though because if you look at OHIP for example it says "you make your primary place of residence in Ontario" as one of the eligibility requirements, but for your green card to remain valid you are required to have residency in the US and spend most of your time in the US.

I am returning to US in January, I was on AOS when I left in Feb we filed for a emergency advanced parole document, which was approved for me to travel back to canada, we choose to come back for treatment here for me to be near family as we have no family in NY only my aunt. I was diagnosed with a terminal illness in NY back in february which we got the options and choose to come back to toronto for treatment to be near family as we needed full time care for our daughter while I was hospitalized for 6 months. In that time my AOS was approved, my OHIP was approved after filing for my presence while in canada for the duration of my stay I still have an address and home in Canada, my husband has been a dual citizen for over 10 years and his always still had OHIP coverage as long as he was present during that time as he travels back and forth regularly, his job was also established out of Ontario, he was an actor and lived here while filming. I am now returning which yea I will forfeit my coverage.

I just looked at the requirement for the Simplified and the General passport. The simplified passport applicant must have the same name as in the previous passport so you are stuck doing the long form. It'll be good to get it out of the way while you are home to make it easier to find guarantors and such.

I didn't mean for my post sound like a personal attack and if that is how it came across then I apologize. You obviously had a very good reason behind coming back to Canada, better for your daughter and better for you to have support around for your treatment and everything was obviously done above board.

I hope that your treatment is helping and that you are doing well.

You have to use the long form and send all the stuff in via mail.

I did mine just recently to get my passport fixed. Got it back pretty quickly. Just follow the directions make sure everythings signed and you have the guarantor.
