Vaccination form from Dr. Seiden in Toronto

Please forgive me all... I'm trying so hard to get these papers put together in the next couple of days...My Vaccination worksheet from Toronto is one page that does not have that official "Number 3025" on it anywhere.

it is titled 'VACCINATION DOCUMENTATION MEDICAL WORKSHEET TWO

For Use in Canada Only

It is signed by Dr. B. Malcolm,and everything is up to date and checks and x's are complete.

so in the bottom, Dr. Malcolm checked off the first two boxes, regarding may be eligible for waivers.

he did NOT check off the 4th box, viaccine history is complete.

So, my question is, has anyone called Dr. Seiden's to request the sheet to be changed to Complete? OR, has anyone submitted that form, without the 693, and successfully completed their AOS based on this form.? I would imagine seiden's will not supply a new form.

Perhaps the official form was already contained in the medical package. :blink:

I have been reading past threads but nothing too recent so please let me know if you were in this particular boat.. thanks so much. :help:

That is the correct form. No need to call to have anything changed on it. If all your shots are current you can submit a copy of that as it is.

If you are missing any shots that were required you will need to get them for AOS and have the 693 done up ( just the vaccination portion )

Your I-129f was approved in 5 days from your NOA1 date.

Your interview took 67 days from your I-129F NOA1 date.

AOS was approved in 2 months and 8 days without interview.

ROC was approved in 3 months and 2 days without interview.

That is the form you need, technically it is not complete if there are vaccines still listed which you have not received, that is why the waiver boxes are checked. I sent mine in with my RFE response without the I-693 without any problems. My worksheet did not have the 3025 on it either.

Hope you are getting a clearer understanding...I know it can be confusing!

That is the form you need, technically it is not complete if there are vaccines still listed which you have not received, that is why the waiver boxes are checked. I sent mine in with my RFE response without the I-693 without any problems. My worksheet did not have the 3025 on it either.

Thanks! I know I have everything required... and anything for ex. hep B that was not done has an x saying not age approriate etc. So I can't see why it is incomplete. At any rate I'm sending it as is and no 693. Thank you thank you thank you. :star: :star:
