OK here I go again. I am filling in 693 and apparently as a K1Visa I only need to fill in page one. But at the bottom of page one, it shows a signature line for a civil surgeon. Do I need to take this form and my vaccination report to a civil surgeon for signing where I live now in Michigan? Thanks!! :star: :star: :star:

I received a RFE for sending in the I693 without a CS signature. If you had a medical completed within one year of filing AOS, send a copy of the vaccinations signed off (my copy was placed on top of the sealed envelope) or a copy of your health vaccination records, a copy of the receipt they gave you for your medical, a short signed note explaining what you are doing. DO NOT send in the I693. Look for earlier discussions on this topic, many RFEs are sent out for incompleted I-693s. Again,even if you send all the above mentioned evidence along with an incomplete I693 you will most likely get an RFE.

As long as you were up to date at the time of your K1 medical and did not require any more vaccines and the medical was within 1 year of filing AOS. If you were up to date at the time of the K1 medical then just send the DS-3025, which is the copy of your vaccines signed by the doctor at the time of your medical, along with the other things I mentioned.

Thanks for answering...So people just don't send in the 693 at all and that works?

If you had all the required vaccinations at the time of your K-1 medical, then you can just send a copy of your DS-3025. If you are missing any vaccinations or had them done after your medical, then you need to have them transcribed onto the I-693 by a civil surgeon.

Thank you everybody!!! I believe I have everything together now except my passport photo's (2 per each ) and my certified marriage certificate... and my HUSBAND :yes::luv: is working on the 864 but he's pretty much there. Thanks so much for the help!!!

I suggest you use: http://www.epassportphoto.com/ for the photos. They're good and cheap.
