I agree that the instructions don't specifically say that page 1 doesn't need to be entirely completed, but the instructions DO say if you entered with a K1 and had a medical exam prior to admission then you're not required to have another medical exam. It also says that if the DS-3025 was not properly completed AND included with the results of the overseas medical exam report then you need to have the vaccination report completed by a civil surgeon, and you submit the vaccination report and page 1 of the I-693.

The civil surgeons certification in part 3 on page 1 is essentially the same as the part 5 certification on the previous version of the form. I don't see any real conflict here except that USCIS has consolidated the applicant's bio section, part 1, with the civil surgeon's certification, which is now part 3 instead of part 5. In other words, only one page along with the vaccination report is now required rather than two.
