The civil surgeon should have a copy of that form. At least ours did. And they shouldn't deny you for being a carrier. They are worried about active cases.

How is a random US Civil surgeon going to have your sheet? -for the post above-

I didn't have the vaccination DS-3025 from the medical I had back home, so my mom called the DR. that I went to before my interview back in Guatemala and they gave it to her, she then sent it to me. You should try calling them to see if somebody can send it to you.

For AOS (K1) you ONLY need to get the vaccination suplement, you do not need another medical exam and the doctor should not even care about it.

Did you get vaccinations at all in Juarez? If you did, you may ask the clinic for the completed DS 3025 form. If you did not, you need proof of having the vaccines (cartilla de vacunacion or whatever you have from Mx).. If you have none of the above you need to go to a CS and get the required vaccines for AOS.

i didnt get any vaccination in ciudad juarez because its not a requirement for K-1 visa. So i waited untill i came to the u.s to get them done wich was 2 weeks ago, thats what i gave to the civil surgeon but i guess since my TB was positive they want me to show my medical exam from mexico to prove i dont have TB thou my doctor wrote a letter saying i didnt have any of the sympthoms. Plus im sure if i had TB they wouldnt have aproved my K-1 visa.

why did they test you for TB if you only needed the vaccines?? Some CS have weird requirements.

If the CS applied the vaccines to you, you should ONLY need the vaccination suplement filled out and signed by him/her, that's it.

Edit: BTW, u did not get a CD from the clinic in Juarez with your xrays? That could probably help...

Juarez doesn't give vaccines to those seeking a K-1, not even if you ask to pay for them. They gave my husband a useless blank DS-3025. I take it they stopped even giving a blank one now and just give nothing. If they would do their jobs there in Juarez and transcribe people's vaccines onto the DS-3025, offer to give any missing, etc. then it would make AOS so much easier for those coming from Mexico. That is just a dream though. Also, I doubt the clinic there will give you anything if you contact them to ask.

The CS shouldn't be messing with tests and TB. They are only to transcribe your vaccinations onto Part 2. They do not fill out any other section of Part 2, ONLY the vaccinations, period. The instructions for the I-693 clearly state Part 1 , Part 2-vaccinations ONLY, and Part 5. Print off the instructions for the I-693 and show them.

Wow, scratch all that. I just checked the USCIS page. The I-693 form and instructions have changed as of 11/16/11. Apparently you need page 1 filled out, and page 5, the vaccination records. Page 1 has a section about your medical summary. They need to know you cleared your exam. This is going to be VERY hard for most people to show, as most people are not given copies of their medical exams from overseas.

I know K1 people does not usually get vaccines in Juarez but I have heard of some people asking for them at the clinic or asking for the medical at POE.. I dont know the result of that since I never asked for anything myself. I was confused with the OP question and mentioning the CS, medical and TB... whatever...

Can't believe the new form requirements, how can you prove you cleared your medical exam? Aren't they supposed to know that already? Geez... I am glad I filed AOS before this ridiculous change.
