AOS question

I am in the middle of helping my wife file for AOS. I am a little confused with the part of form I-693. It says if test were done before hand then you do not need to have a test done again if within one year. Do we still fill out something on that form? Also it always talks about form D-3025, but all my wife has that was given back to hear is like a card showing all the vaccinations she has had. Is that all we need to send it is a copy of that or do we need to have the form filled out by a civil surgeon to?

if she had all the comleted vaccinations at the time of the first medical then you could just send a copy of that card, is all i did, if you are within that one year you do not need to send ay of the i-693. You do not need a civil surgeon at all as long as it was within 1 year and all the vaccinations were current and correct

I didnt not submit I-693 because in the instruction said that K1 doesnt need that..but then i got RFE asking for I-693 to proof Varicella vaccination.. so then we sent the RFE response for I-693.. here my thread

Is it okay to just send the card in? She did not get back form D-3025?

i think if your wife has a vaccination record state that she has varicella vaccine then you can just attach the copy of vaccination record..and thats will be just fine eventough you dont have the DS-3025.. if she doesnt have any records then better prepare the I-693 (thats if you dont want to wait some more time to respond RFE) if you dont mind to PROBABLY get RFE then it is okay too just send vaccination record to them (eihter has varicella or not).. I got RFE but whole process just takes 4 months so its not a big deal..

I asked the clinic where i got the medical in Jakarta about the copy of my medical result which DS 3025 was included. They sent it by email. I printed it out and attached it on AOS forms.

I finally have all the forms filled out. I will make copies of all today and then send it out. Are the fees $1070 and can I write 1 check for the total or do you have to break it out between the filing fees. Also what address will I use to send it to.

Yes $1070. One cheque is fine. Address is in the instructions and also in the AOS thread in my signature.

Yep yep same tracking site where you enter the receipt number on uscis.gov. Thanks, what number can I use to be able to check my status on all the forms? Is it the same from the NOA1 number?

It's the NOA1 number from AOS
