I was curious if I needed to complete form I-693 when I have already completed my medical at Woking Medical in Vancouver.

I already have a vaccination supplement and all the requirements that were needed on it as required on page 4 point 8 on the I-485 instructions.

Would I just need to complete the I-693 part 1?

Thank you for your help everyone

No. No medical. NEVER EVER EVER "only" complete part one. It WILL result in an RFE. The I-693 MUST be signed off on by a CS.

Nope, as long as your medical was within the last year you'll be fine

I have an interview appointment on November 17, 2011, but was confused when they asked for an I-693.

Form I-485 instructions: Page 4, Point 8 says that I do not need to

Also Form I-693 instructions:Page 4, FAQ (Section III, Point 2) does not ask for an I-693 when the DS 3025 (vaccination supplement) was completed properly.
