Supporting evidence for 1-131 re entry!

Hey everyone! Hope all is well! I am currently filing a I-131, as my wife and I are going traveling, so just incase we are filing the re-entry permit in the event our stay abroad lasts longer than 12 months. Hoping to file it tomorrow, but got a little confused. All it says, that I need front and back copy of I-797, front and back copy of green card, and filing fee for the permit..... So just a couple of questions.... should the filing fee and biometrics fee be on separate checks?....Do i need to 2 passport size photos( It doesnt mention them, for someone filing inside the US, however, we have sent photos away with pretty much everything else before!)...and what should i write in the space, where it asks for category?would that be K1 or CF1 as stated on my green card???

any help would be amazing! please let me know if i left anything out!If anyone has any questions about this, then ask, i think i know everything else about this, except for the above parts!

I think you did good. To be honest, I am not sure if the pictures are required or not. I have sent my I-131 application and did not include the photos. When I read the instructions it does not mention about photos for re-entry permit for GC holders. I think it talks about photos for AP and other application types. May be I have to call them and find out.

Hey, thanks buddy! I called them, and they said photos are not required. When did you mail yours in?

I sent mine last week to Dallas, TX Service Center. They received it on Sunday 11/6/2011. I have included the electronic notification request along with the application. This morning I got an email and txt message on my phone informing me they have forwarded my application to Nebraska Service Center and I should recieve the NOA in the mail in the next 7-10 business days.
