Based on what you stated you can apply in January of 2012. You can apply no earlier than 90 days prior to your 5 year green card anniversary. Look at the issue date, add 5 years, subtract 90 days (not 3 months). That is the earliest that you can apply.

Read the Naturalization Guide here on VJ as well the N-400 section on the USCIS website to learn what you need to submit.

Use the USCIS date calculator found here: http://www.uscis.gov/dateCalculator.html

When the time comes for your application you will need to submit

1. Filled out N-400 form

2. Photocopies of front and back of your greencard

3. Two passport-style photos

4. Check for fee

Read all the N-400 instructions to make sure that you are not missing any other documents. Tax transcripts can be ordered from the IRS but they are not needed for the application; however, I would suggest to order them anyway and have them with you during the interview in case the officer asks for them.

May also want to bring proof you paid your parking and minor traffic violation ticket fines, even from 40 years ago. Seems to be the latest bug.
