WHAT EXACTLY THE REQUIREMENTS NEEDED TO GET A MEDICAL CHECK UP AT ST. LUKES

Guys, please share... Ive been asking in forums for K1 visa about this and got different answers...Fiance and I are planning to get a medical check this month .. we already have our MNL3 and NOA2...but I here, on my part have not receive yet that packet from Manila Embassy... on that forum some answered its ok no packet yet as long as i have with me the copy of MNL but some say, should have that packet also ..

You guys who had their medical check up at St LUkes, what exactly are the things needed/ docs needed there?please help...

Register online at St. Lukes webpage (WWW.SLEC.PH), print 2 copies of the online registration, you will present this to the lobby guard at St. Lukes to furnish you a fill-up form, bring 3 2x2 pictures, a valid visa, medical fee payment. USE THE MNL CASE NO. when you register online. I had my medical examination last Oct. 10, 2011. Good luck!

For my experience here is the list of documents I submitted:

1. original passport and 1 photocopy of it

2. NVC letter with the MNL case number. (scanned and sent to me by my fiance through email)

3. SLEC online registration form. downloaded, answered and printed.here's the link:

4. 3 pcs US visa picture - white background, with both ears uncovered and without earrings.

5. copy of the appointment letter for the interview from the embassy

6. Fee for the medical

if applicable:

- Annulment Papers

- copy of past communicable/serious disease from the health care institution you were admitted or treated (if applicable)

Things to bring:

1. ballpen

2. jacket

3. bottled water but there are water fountains in each floor level

4. hair tie/pantali sa buhok. but they have their own hair clips. this is for the xray.

this were the things/documents i brought with me. let's hear from the others. btw, my medical was last oct. 3, 2011.

in addition to renecherubs reply above:

1. Do not worry if you did not receive the NVC letter yet as long as you have the MNL#.

2. Do not worry if you can't get online and pre-register for your appointment, you do not need to. Pre-registration helps a lot for you to be able to start right away your medical check-up once you get at St. Luke, but you can also fill up the forms once there.

Well just for some added info, we still have not received the packet from the embassy, and we have already completed medical and the interview. So go get your medical done and schedule ur interview, and prepare the docs required.

Tahoma.....is it best to Schedule Interview First or do medical then Schedule?

Since Chinook had to travel from Bacolod to Manila, we scheduled her interview first so that she could complete the medical and the interview the same week.

For those who live in or near Manila, why not get the medical out of the way as soon as possible? It can be helpful to start the medical early because you just might have to wait for that eight-week TB culture. :(

You Can Pre- Register onlne now, and just print it out and take it with you on the day of the medical along with the other Documents that they list. Good Luck!:thumbs:

What if my fiance doesn't have a copy of his vaccination record? Will this prevent him from passing his medical?

No worries. A vaccination record is optional.
