As you know, the K-1 visa allows us to bypass the medical exam, since it was performed prior to the K-1 interview, and just get the vaccinations performed and/or certified for the change of status packet (I-485).

Instructions for I-693 clearly state that in our case we news part 1, part 2 (vaccination table only), and part 5 filled out and signed. However, we just got the vaccines done and the doctor REFUSED to fill out and sign part 5. He said that it was legally obliging him to perform the entire exam, when all we got were vaccinations. They DID sign the vaccination table in part 2.

Has anyone encountered this? Was it a problem? Are we going to be rejected because part 5 is blank? If so, advice?

Yes. It's useless if they don't sign it. The I-693 explains what they're signing. Unfortunately you got a CS who doesn't know their job.

Well they are willing to sign the bottom of the vaccine table in Part 2, and they say that they've never had any prob with that in the past, but I get the impression that the doc doesn't really know about the administration of these forms. Thoughts?

Should I then take the new vaccine histories from this bunk doc and look for a CS that will simply sign part 5 of the form? Does it have to be in a sealed envelope from the doctor when I finally send it in with I-485?

Signature at the bottom of part 2 will not do. Find another CS (someone knows what they are doing). And yes, it has to be in a sealed envelope.
