I wanted to bump this up. We finally have the marriage license here in the USA. Is there some sort of actual form for affidavits that says we've been dating/together?

No form - you just include them with the bonafide evidences.

July 2005 - met my awesome, hot, amazing love in Lousiana.
July 2006 - Married said love and moved to Canada.
June 2011 - Entered US to visit family, decided to stay.
Feb. 2012 - Sent paperwork to Chicago.

May 2012 - Received green card.

Affidavits from my parents, her roommates, my friends, etc. that's no problem. I can't put her on my insurance until we're officially married here though.

You can't file I-130 until after you're married anyway. Most people get married, start cohabitating, become financially interdependent and gain other evidence of their relationship, and then file I-130. Honestly, you are putting the cart before the horse by working on your I-130 before you are even married.

To be clear, to adjust her status you are filing I-130 and I-485 with the supporting documentation. I-765 gains a temporary employment authorization document until the Green Card is approved. I-131 is used to request an advance parole, which allows her to travel outside of the U.S. and re-enter the U.S. Otherwise, if she travels outside of the U.S. the adjustment application is cancelled and she is most likely barred from reentering the country. @Iscariot: what I heard from many friends bona fide marriage is not a "must have bank account together, must have insurance together, must live with together" etc...it is good to have, BUT! It's not the end of your journey! I have many many friends who are in a religious group that "prohibits" cohabitation prior to marriage, or any sort of "joint" account...hence these friends of mine all went along without "joint bank account" or without "joint health insurance/car insurance/PG&E bills" etc. and neither did they live with their boyfriend/girlfriend. When they were asked how come they didn't live together, or how come they had nothing joint before marriage they were just simply honest and upfront that their religion does not support that and they got their green card just fine. It is unconstitutional for rejecting someone BASED on their religion and they can well tell if your marriage is true regardless of how long you guys lived, or did not live together prior marriage, or not having a joint bank account etc. Don't worry, if you are having a true relationship, hence a true marriage in the future, you guys will be perfectly fine! Let's not these things scare you! They will see very well if you guys are meant for each other! :)
