rfe form I 693

hi vj members, today i received a notice letter form uscis a request for eveidence.. we filed aos for k1 anf k2 sept. 29 ,2011, we did our form I-693 august 22, 2011. this i what is written in the letter;

-- you submitted form I-693,however the civil surgeon certified the exam more than one year before the date the applicant filed the form I-485. The form I-693 must be dated within one year of filing form I-485.

--now i looked at the extra copy the CS gave us, ive found out that my form I-693 has no signature and date, but my son's I-693 has a date and signature, maybe that's the reason i got rfe, and my son doesnt get rfe at all.

what should i do? should i go to the CS ask them for another sealed I-693? that is their fault right?

If it's been within a year, I would contact the civil surgeon, tell them the issue, and ask them to give you the documents again to send. Be aware that if you send something incomplete again, your application will be denied, so be very clear with the CS.

Yes, that is why you got RFE. Go back to CS, bring RFE letter and ask for another copy, make sure it is properly filled out, dated and signed. It was their fault so they should do it for free.
