ADVICE NEEDED! #12 on 129-F

Hello!

I am still looking for help with #12 on the 129F!

My fiance currently works on a cruise ship, therefore I will say 'YES' to #11 and put 'CREWMAN' in the open space of #12.

The part that Im questioning is the Arrival/Departure Record (1-94) Number...

He doesnt have an I-94. He does have an I-95 so I will write his arrival date and date of expiration...but do I need to put a number in the opening for I-94 number? The number from the I-95 doesn't fit properly within the open spaces...so Im thinking that I leave that blank considering he doesn't have an I-94?

What do you think? Does anyone have experience with this? If not, any advice is appreciated!

*Is there any reason why I should include a copy of his C1-D visa?

*04/26/12 - Carlos Medical Exam

*04/27/12 - Carlos receive Interview Binder from me

*05/04/12 - Carlos Interview 8:00 AM (Has to return with two more documents)

*05/14/12 - Carlos return to embassy with missing documents - VISA APPROVED

*05/18/12 - Carlos picks up VISA at Embassy

*05/19/12 - POE MIAMI

Hi. I have the same issue, I was wondering what you ended up doing? Did you leave it blan or did you put his I95 number? Thanks.

3-01-12: UPS Delivery Confirmation Received

3-06-12: NOA1 Text/Email - Application has been sent to Vermont

3-09-12: NOA1 Hard Copy Received

Yes you put the I-95 info in there (it states that on the form, says I-94 or I-95)

I just put the date he arrived in the US, and the expiration date of his i-95...I did not include the numbers that were on the i-95 because they do not properly fit into the boxes provided...note I have not yet recieved NOA2 so I cannot tell you if it worked or not! :)
