My i-94 is in my dad's name. My DOB is correct. Will this be a problem?

I am going to apply to adjust my status and noticed that my I-94 is in my father's name with my date of birth. I looked at my dad's i-94 and his is in his name also with his date of birth... the departure numbers are different on each card. We came to the U.S. together when I was a child... how could the immigration officer miss this?

Will this be a problem if I submit my adjustment of status forms with a copy of my i-94 in my father's name?
