I am 46 but there are no records for vaccinations I have had so I need some again and some I have not had ...is there an official form I need to get the surgery to record these on??

Married - 06/26/2012

AOS - 07/31/2012 - 2months 18 days

Process from filing 129-f to AOS approval 349 Days

do you mean if you get your shots done at the doctors before you go for the medical?? if so then from what i can work out from other posts you just get the doctor to do a signed print-out of all the vaccinations you have had on surgery headed paper

AOS approved 3/29/13 after almost 10 months of waiting. No RFE's and no interview.
