Medical Examination Form (Form I-693)

We got our AOS appointment letter, and one of the items listed to bring was the Medical Examination Form (I-693), unless already submitted. My then-fiance came over on a K-1 visa, thus already did a medication examination with a package, etc. So, is that the form they are referring to, and therefore it is already submitted, or what? This was answered in the archived threads yes, the form submitted then was the I-693. And provided AOS is filed within one year of it, a new one is not necessary. However, a vaccination supplement was mentioned.

I don't know whether we have a copy of the I-693 - do we need that for a vaccination supplement, or can they do the vaccination supplement regardless?

What if my fiance had the vaccination supplement taken care of by a physician in her country before leaving, but not a USCIS physician?

Join the club of some very confused people. I just submitted my AOS paperwork and my wife (then a K1) was never given any copies of the medical. It was all put into the infamous selaed envelope.

We cannot comply with supplying the supplement if we wanted to!

There's clearly some idjit at USCIS that's uninformed and ignorant of the process.

I did not submit the supplement but rather included a letter that siad that I *cannot* submit unless I have the medical info that was denied my fiancee to begin with!

Let's see if I get an RFE........

We did not submit the supplement either. At the time, we did not know that the vaccination supplement form was needed for AOS and the civil surgeons in my husband's country don't give out that information at the time the medical is performed (they just send it all directly to the consulate). We put in a letter of explanation saying all the results were surrendered at the POE. We'll let you know how it goes as we never got an RFE and have the AOS interview on 3/8.

Your description is correct. They may or may not ask for the supplement at your interview. I would get it just to be safe. It needs to be filled out by a USCIS approved Civil Surgeon.

I found a vaccination form my fiance got from the USCIS physician, that has one listed as "insufficient time interval" thus "vaccination not medically appropriate." She had her regular doc get it done a week later, but of course, not a USCIS doc. Oh well, we'll try to get it done before our 2/8 AOS appt (either duplicate the vaccination or have a local USCIS doc sign off), but otherwise see how it goes.

Ah, just figured something out, thought I would post it for others. On the USCIS website FAQ regarding medical examination, it says:

What form is needed for the medical exam?

If you are applying for adjustment of status in the United States, Form I-693, Medical Examination of Aliens Seeking Adjustment of Status, is used to report the results of the medical exam to USCIS. If you are applying for a visa at a U.S. Consular post overseas, Form DS 2053 (Formerly Form OF 157) will be given to you. The Consular officer will supply this form and the accompanying supplements, and will give you the names and telephone numbers of Panel Physicians in your area. There is also a vaccination supplement which your Civil Surgeon or Panel Physician will provide. It is used to record the results of the vaccination assessment.

Though my fiance was not given a Form DS 2053, she did get a copy of the Form DS3025, which is a vaccination documentation worksheet that says "For Use with DS 2053". That's something I guess...

Timeline

I came in on K-1, and all the medical info was submitted at the POE. I took my vaccination record worksheet to a doctor who filled it in for $25 and put it in a sealed envelope. If your fiance had the vaccinations done at the american embassy in her home country then she should have gotten a record of her vaccinations which you take to the doctor. Otherwise then you can just go to the doctor, have the vaccinations done and get the supplement form filled in then.
