AOS RFE I-693 Varicella Vaccination incomplete

Hi I just want want to share my experience with my AOS process..

I got RFE for my AOS about varicella vaccination incomplete. back when i had medical check up in Jakarta with Dr. Narain Punjabi, he asked me if I had chicken pox before and i told him I was sick (chicken pox) in 2007. then he put check mark on the vaccination sheet (DS-3025) and year (2007) also MMR ( I got MMR shots) I never had vaccination shot before and and no records at all.. so all other vaccine marked not age appropriate.. then on #2 result he put Vaccine History incomplete. Applicant may be eligible for blanket waiver(s) because vaccination(s) not medically appropriate..

I attached DS-3025 and my MMR vaccine card to the packet for AOS, then I got the RFE requesting varicella vaccine. I went to civil surgeon as they suggested and I told them that I had chicken pox 4 years ago and I had no written proof for that. then He said i don't need the vaccination. He put VH (varicella History) on my I-693 and result is vaccine does not meet immunization requirements.

I feel horrible when i looked again on my DS-3025 and Dr Narain didnt marked VH on the varicella tab which maybe will not giving me RFE (I can save my $90 civil Surgeon bill and save time!...) :bonk:

oh well, so if you choose him as the doctor who run medical check up on your k1 visa remind him to give you varicella vaccine or put VH if you had chicken pox fever before...

:ot: anyway, does anybody know how to send back RFE? USCIS gave me a small (DL size or regular business envelope) envelope with address on it and says send the I-797 E (NOA RFE) and the civil surgeon report on it. the result from civil surgeon should be sealed and never be opened but its on C4 size envelope (as big as A4 paper).. is that mandatory to use the envelope that USCIS sent to me? or it doesnt matter?

We had the same thing and we just went and got the varicella shot (despite the fact that in Serbia, all kids get this when they are young, but we had no record of it)

Send it the same way as you sent the AOS package but make sure you write the correct address (the one that is written on that little envelope they sent you)

Hi Nenooye -

I know this threat was few years old. But now I have a similar situation. My wife was having medical exam in Indonesia for K-1 Visa. The doctor did ask her whether she had a chicken pox before which she answered "Yes". But the doctor didn't put "VH" on Varicella History on DS-3025. There is a checkbox on the area "US vaccination requirements COMPLETE". The exam was also done in Indonesia. I am wondering if some doctors over there are lack of knowledge on how to complete the form. What is the point of asking your patient if you didn't put the answer on the form?

I am asking someone in other post if I can still use it for filling AOS or I need to go to CS for a correction on I-693 form.

01-23-2008 = Consulate gets file; outdated Packet 4 mailed to fiancee 1/27/08; rec'd 3/3/08.

04-29-2008 = Fiancee's 4-min. consular interview, 8:30 a.m.; much evidence brought but not allowed to be presented (consul: "More proof! Second interview! Bring your fiance!").

05-05-2008 = Infuriating $12 call to non-English-speaking consulate appointment-setter.

05-06-2008 = Better $12 call to English-speaker; "joint" interview date 6/30/08 (my selection).

06-30-2008 = Stokes Interrogations w/Ecuadorian (not USC); "wait 2 weeks; we'll mail her."

07-2008 = Daily calls to DOS: "currently processing"; 8/05 = Phoned consulate, got Section Chief; wrote him.

08-07-08 = E-mail from consulate, promising to issue visa "as soon as we get her passport" (on 8/12, per DHL).

08-27-08 = Phoned consulate (they "couldn't find" our file); visa DHL'd 8/28; in hand 9/1; through POE on 10/9 with NO hassles(!).
