Do I need the I-693 completed or vaccinations okay?

And would that change your day to day life any? If you want to spend the extra money to avoid delaying your greencard a couple of weeks, then do it.

I thought you got an MMR shot and a Tdap shot just before your visa. If you haven't had any shots for years, then you need some and this discussion isn't important. If you did have shots before the visa read on. MMR is given in several doses. They can't give them closer together than 4 weeks. So I thought you meant at your medical, it was too soon to get dose 2 so that's why your form says insufficent time. You won't start over if you have a record of having one MMR dose. They can give you another dose if 4 weeks has passed. But you don't have to ever get that second dose or a flu shot if your form was filled out and has what I wrote in post #8 and you use your St Lukes form instead of going to a civil surgeon. You can be approved without it. But if you go to a civil surgeon, then he will probably give you the shot. By the way, you were checked for TB at St Lukes, so getting the TB test wasn't necessary.

You could get an RFE at the interview. You can get one up to the last minute.

Sorry you don't understand the immunization things. I can't explain it any better, so do what you feel best doing. I'm not trying to talk you into anything. I was just providing information I have gathered from many official sources. I don't have time to write out 10 pages.

I can't help but to jump in here and tell you how much I appreciate your painstaking explanation of the issues regarding DS-3025 and I-693.

I've read many of your posts regarding this issue, including your reply to my posts.

My fiancee has since filed her I-485, had her biometrics done 2 days ago, and her case has been transferred to CSC last Friday. She did not see a CS for a I-693 and no flu shots either since she filed in September. We submitted her DS-3025 that came in a sealed envelop provided after her vaccination in China. She also sent in photocopied pages of her "yellow book" vaccination record issued by the medical facility.

Just in case someone is curious as to how I know it's the DS-3025 in the seal envelop, my fiancee called the medical facility in China and was told that's what it is.

we were never asked for it but i provided it anyway and she opened it and put it in the file without even looking at it.

just in case we have a copy of it also which should have been done when you first go to the medical. LOL

and she was approved

I realize you have already received the transcription so yes, it is always better to be safe rather than sorry. I did want to share that with what you reported on her Panel Physician Immunization Worksheet - that she had all of the medically relevant shots and may be eligible for a waiver that you could probably have just submitted the worksheet if you had not found a co-operative Civil Surgeon.

I had exactly the same notation on my vaccination record, and submitted the AOS without the I-693 Vaccination supplement. My AOS was approved without me needing to provide the I-693 Vaccination supplement and without an interview. Basically the 'may be eligible for a waiver' means that your fiancee had all of the necessary vaccinations for her age and time of year and could be 'waivered' from requiring more vaccinations.

Some have been successful in submitting the worksheet only and others have received an RFE to submit the I-693 after all. It doesn't seem to be consistent so getting the transcription done is not a bad idea even with the 'waiver'.
