HI everybody, I need some advice about I90 I hope someone can help me with this,I filed I90 form to correct my middle name on my green card which they sent me wrong details...then after 2 weeks they return my document with rejection notice saying like "The applicant did not properly mark Part 2 question 2 providing a clear reason for applying. I checked my form I ticked the box letter D under Section B letter D. so what are the right box to tick?

The next paragraph its says: Please be sure to complete fully, submit the approriate fees and include all required supporting documentation.

My question is was that means I need to pay the fees? even it wasnt my mistakes at first?

and what are the required supporting documentation, I sent then marriage certificate, birth certificate and my state ID and employment authorization with correct details on it. and the incorrect GC plus the G325A, passport, I94,NOA welcome notice so what more should I send to them? I hope someone can answer my question i will appriciate it

05/02/2022 St Luke Medical appointment

05/23/2022 Sputum test wait 2 months to released result

07/22/2022 Sputum test passed same day Immunization

07/27/2022 Interview US Embassy Manila (Refused ask Embassy request DNA test)

08/31/2022 DNA test Kit Paid (Universal Genetics)

9/13/ 2022  Embassy took the sample both me after 3 days result was 99.9999 % Positive :)

9/29/2022    Passport with visa 

10/2/2022    CFO registered online only  

10/2/2022  Paid 220 for green card

10/19/2022 Flight to USA 

10/20/2022 Arrived in Alaska USA

hi gray, on your application forms for the permanrnt resident are you sure you put the correct spelling of the names on the forms,if you put the wrong name then it's your falt you pay the fee for the correct name change,if it is there error then no fee.on the 1-90 form you see it says, reason for application, then you put my card was issued with incorrect information because of a uscis adminidtrative error, then put a check in the box d.and send evidence of the error,you need to send the green card back first make sure you make copies of all you send them,

actually they put my moms maiden name as my middle name instead of my maiden name, I need to retain my father's surname as my middle name but they put my moms maiden name. i did sent my incorrect GC and the document that I mentioned above so I dont know what more do i need to send i made an info pass before sending those document and they just told me to send birth certificate and marriage contract so i did i just dont understand why do i need to pay again since it wasnt my fault?

is it possible to change it or file I90 before the 90days expiration of my 2 yrs GC?
