My wife has had about half of the required immunizations, but they were done in Lithuania and her shot records are in Lithuanian. Can we just have her translate on her own what they are, or would they need to be officially translated? Has anyone else ever encountered the i-693 examination with foreign medical records?

I would get them professionally translated if your planning on bringing them to the US - some civil surgeons are picky.

Umit's vaccination card (Turkish) was accepted - but it had English/Turkish on it.

My Advice is usually based on "Worst Case Scenario" and what is written in the rules/laws/instructions. That is the way I roll... -Protect your Status - file before your I-94 expires.

WARNING: Phrases in this post may sound meaner than they were intended to be. Read the Adjudicator's Field Manual from USCIS

The vaccination record is usually used by the CS to transcribe the vaccinations. It is also usually customary to send the source document with the medical packet. Most CS will want to do this.

Read the "Translations" section in the instructions of the form and it will tell you what is needed for an acceptable translation.

Try to find a CS in your area that speaks your wife's language. That worked for me. I live in MD and here all the CS I've called couldn't read my immunization records in Russian and were telling me just to pay around 400$-500$ to start the shots thing all over again. I didn't want to spend that kind of money, so I found a Russian CS in NYC and just took a bus there. Being Russian, the CS had no problem reading my immunization records and I had my full medical done for 160$. Even with taking the bus back and forth I've saved around 200$.

Depending on how old your wife is, she probably wont need many of the shots listed on the form. My parents only needed 3 of them (they are over 50). We got a list of the shots they needed and simply went to the local health department and had them done for $15 a piece and took the shot record back to the CS and they included it in the packet.

This might be easier than looking for an CS that speaks your wife's lenguage or paying someone to officially translate.
