Dear all,

I use EP to apply green card for my mom, she will go to Guangzhou for interview next week. It's not easy for her to fly to Guangzhou, so I just want to make sure she bring all the documents with her.

The following is the document checklist, would you please help me to check if I miss something, thanks:

Except of visa appointment letter, they are:

1: DS-3032 Form (with her signature)

2: DS-0230 Part I and Part II Form (with her signature)

3: Package 3 Supplement Sheet (with her signature)

4: I-864 Form

5: W-2 Form and Tax Return (year 10, 09 and 08)

6: Pay Subs (Most recent 9 pay subs)

7: Employment Letter

8: Bank Statement

9: Two Passport Style Photos (more pictures for medical exam)

10:A photocopy of passport’s personal information pages

11:Expired Passport

12:Visa Extension Application in USA (my mom extends her visa in 2002)

13:Visa Extension Approval Notice in USA

14:Family Photos

15:Birth Certificate (for my mom)

16:Name Change Certificate (for my mom)

17:Marriage Termination Certificate (for my mom)

18:Police Clearance Certificate (for my mom)

19:Birth Certificate (for me)

20:Marriage Certificate (for me)

Also I already pay AOS fee and IV fee in USA for my mom, except fees for medical exam, EMS fee if approved, is there other fee for visa interview?

Thank you very much!

I check my mom's visa appointment letter again, the document table is as the following:

CORRESPONDENCE INSTRUCTIONS C

1 - IMMIGRANT VISA APPLICATION PART 1 S

1 - IMMIGRANT VISA APPLICATION PART 2 S

2 - PASSPORT Y

3 - BIRTH CERTIFICATE S

4 - ADOPTION F

5 - MARRIAGE CERTIFICATE F

6 - MARRIAGE TERMINATION S

7 - COURT RECORDS F

F - FINANCIAL EVIDENCE S

8 - CUSTODY PAPERS F

9 - MILITARY RECORDS F

10 - CHINA - MAINLAND POLICE CERTIFICATE Y

FEE PAYMENT $

PHOTOGRAPH Y

F - PETITIONER MARRIAGE CERTIFICATE S

F - PETITIONER BIRTH CERTIFICATE S

GUZ P3 SUPPLEMENT Y

I have 2 questions:

1: the first line "CORRESPONDENCE INSTRUCTIONS C", what's this? do I need to bring this to interview? if yes, where can I get it?

2: in line "F - FINANCIAL EVIDENCE S", "F - PETITIONER MARRIAGE CERTIFICATE S" and "F - PETITIONER BIRTH CERTIFICATE S", what's the meaning of prefix "F" character?

Thanks!

that's the appointment letter - further down in the appointment letter there

SHOULD BE

a cross reference with what the 1 LETTER CODES mean.

Look again, see if you can find it.

Sometimes my language usage seems confusing - please feel free to 'read it twice', just in case !

Are you Naturalized? You may need your naturalization certificate. I have heard of people that they were asked about it beacuse they could not find it in their file. Extra passport pictures just in case.
