Hey everyone!

I've got two questions for you today... (The first one is really short so I figured I'd just put them in one post!)

I have my Biometrics Appointment on Monday, and on the paper they send you it says to bring a valid identification document. I filled out all forms with my new name, but I don't have any documents (passport, drivers license etc) with that new name. Will it be enough to just bring my passport with my maiden name and the marriage certificate?

Now to my second problem, which is a lot more complex...

I received an RFE a couple of days ago, asking me to send in a "properly completed and signed form I-693, Report of Medical Examination and Vaccination Record". Thing is... I DID the medical exam and I sent it in (in a sealed envelope saying "for USCIS use only", as requested), the Medical is not more than a year old (I took it in July) and the doctor was a designated civil surgeon.

So... I don't really know why they sent me an RFE. I called the National Customer Service Center, but all they could tell me was that if they sent me an RFE, that means that I should take the medical exam AGAIN and send the results in. But honestly, I don't want to take it again! Besides paying 110$ for it, I didn't have a very nice experience with the exam (I fainted when they took my blood pressure after giving me the TB shot :( ) So I was just wondering what your thoughts about that are. I've been thinking about what could be wrong with my medical, and the only thing that came to my mind was that I had the medical exam BEFORE getting married, meaning it has my maiden name on it, but I filled all the other forms (I-485) out with my new name. Do you think that is the problem? But then again... isn't there a field on the forms asking you to put in "other names used"? I definitely put my maiden name in there. And even if that field wasn't there, the people at the USCIS should be smart enough to figure out that the person in the medical is the same as the applicant?!

As you can see, I'm pretty confused as to what to do about this. Yes, the easy way would be to just do the medical again, but I just see this as a waste of money and time. So I just wanted to hear what you think about this!

When you had your medical, were you given a DS-3025 (record of vaccination) ? If so, you will need to seek a civil surgeon to transpose that record onto the I-693 as part of your AoS. This doesn't require a full examination. Just make sure when you contact them, you are VERY clear about only requiring the vaccination record be transferred onto the I-693 or they will attempt to book you a full medical again. As civil surgeons are unregulated on cost and charge hugely different fees, you may wish to shop around if there is a choice in your locality.

Some people have been successful challenging the requirement by simply sending in the 3025, but most people who do not submit the transposed vacc record generally get an RFE.

Edit: I also took passport and marriage certificate as ID for the AoS interview.

Thanks for your fast answer! So I just got home and looked at a bunch of papers that the doctor gave me after my examination. All I have is the first page of the I-693, page 4 that contains "vaccine history transferred from a written record", and a paper with the lab results of my blood test. So... I don't have a DS-3025... What now? Where do I get one?

I'm not sure about the medical thing.

I can however, answer your first question. When we went in for my Biometrics (we did a walk-in), they didn't care about the names matching up. I still have my passport and license in my maiden name, but filed everything under my new married name so my notice was in my married name. I did take our marriage certificate in case there was an issue. Hopefully they don't hassle you about it either.

09/07/2011 - Still no Biometrics letter. Called to request Biometrics appointment.

09/26/2011 - Interview appointment letter received, scheduled for 10/27/2011.

10/03/2011 - Biometrics appointment finally received.

10/07/2011 - Successful Biometrics walk-in completed.

10/07/2011 - Text and email notification, EAD Card/Document production.

10/17/2011 - Received EAD in the mail.

10/27/2011 - Interview - APPROVED on the spot

So I've been doing some more research on the whole thing. The form you were talking about Brit is the I-693A, right? So does this always need to be sent in with the medical, even though the information on there are basically the same as on page 4 of the I-693? Like I said, I only got a copy of three pages from my doctor, and that didn't include the I-693A. But I also know that there were definitely more pages in the sealed envelope he gave me. So no way for me to know if he sent it in, right? Or does the applicant have to make sure that the additional form is being sent in? I'm just confused, the doctor we went to seemed to do a lot of exams for visa applications, so you think he'd know what he's doing.

Also, what if I get my records transcribed onto an I-693A and send that in, but that's not what they wanted... Will I get another RFE or is there gonna be any other consequences? (Kind of like... We gave you a chance, thats it!)

It would be awesome if someone could help me out with this! Thanks :)

I would contact the doctor and see if they can tell you exactly what was given to you in the sealed envelope. When you respond to your RFE, you need to make sure to include EVERYTHING USCIS asks for. Better too much than too little, because if your response is inadequate or missing something you will NOT be given another RFE. Your case will be denied, and you will have to file a motion to reopen it.

Thanks for your answer! That would have been my next question, what happens if you send in the wrong thing. My RFE says to send in a properly completed medical exam and vaccination record... So in that case, I'll probably just have to take another medical. Lame :( I definitely don't want them to deny my application. Thanks for your help!!!

Thanks, kampai99!!! Good to know I'm not the only one that happened to! Do you remember by any chance what exactly it said on your RFE? Cause I've been wondering if they lost it, or if there was something wrong with it.

Thanks, kampai99!!! Good to know I'm not the only one that happened to! Do you remember by any chance what exactly it said on your RFE? Cause I've been wondering if they lost it, or if there was something wrong with it.

Did your civil surgeon just copy the medical or did he fill it all out again? And did he charge you for that? Let me know how it all works out for you :)

It said exactly the same thing as yours (Send a signed and completed from I-693...). My CS filled it out again with the same lab results since they were less than a year. He didn't charge me a penny (thanks doc!). Your CS should do the same if your results are still less than a year old. I mean it doesn't cost him anything except few pieces of paper and 10 minutes of his time, right? Oh and make sure your CS use the right version of I-693 form.
