hi all my sons medical is on tues, we had trouble with his AU passport AND ITS now on it way express to him my question is what else do we bring besides the money ,photos (3),himself ,vacinne results,??? do i bring the DS-2053 forms for do they supply them?

I went to the same doctor. The doctor supplies all the forms. Tell your son to go there a little early (maybe 15-20 minutes) so he can start filling in the forms - he will need to fill in some of the information himself and it takes a while. He will only need passport, payment and any vaccination records he has.

2013-08-20 (DAY 8) Noa1 received via Text around 11pm, next day was email. Received my case numbers

2013-08-23 (Day 11)- received hard copy in mail for Noa1 gc , Work permit

2013-08-30 (Day 18) Received Biometrics appointment letter in the mail.

2013-09-17 (Day 25) biometrics appointment day @2pm Detriot

2013-09-25 (Day 33) received text and email , I485 changed to testing and interview

2013-10-02 (Day 40) received text message and email @7am about my interview date

2013-10-04 (Day 42) Received hard copy interview letter in the mail

2013-10-10 (Day 48) Work permit Card in production

2013-10-16 (Day 54) 2nd Text and email @830am Work permit in Production

2013-10-21 (Day 59) text/email notice 10am . Work Permit sent

2013-10-21(Day 59)Text/ email @9pm received text with tracking number -2 day delievery

2013-10-24(Day 62) received my work permit in the mail.

im in the States but nick is going to see Dr. Charles Okraglik in Port Melbourne , there was no mention on forms so i guess they supply them?

Yep I went there. He might need his NVC case number (there's a spot for it on the forms).

Not required for the medical. If the OP's son wants it expressed posted he will pay the express post fee when paying for the medical.

OP - also. Remind him not to pee as he'll need to give a urine sample.

Vanessa is correct - express post envelope is not needed. They will ask if you want it mailed (and add $5 to the cost of the medical for the express post envelope) or you can elect to pick it up when it's ready (about 5-10 working days).

I don't remember needing the case number but I guess it wouldn't hurt.

I had mine done with him in July, i paid the five bucks to get it posted to me and it only took about a week as i recall.....check my signature though, it's on there, lol. Also, i didn't have my case number either, NVC didn't have our application in their system yet and it wasn't an issue. I thought that i would be required to do the urine test but i never got asked to in the end, only had some blood taken and that was it :)

I know this is a bit off topic..but while i'm here.....i have my copy of the DS whatever number it is, my vaccinations form but i noticed that i accidentally wrote on it that i was born in 2011 :/ it never got bought up at my interview or anything, is this likely to be an issue during AOS??

Finally time for AOS.......

12/09/11 AOS package delivered

12/14/11 Received NOA for I-485 and I-765 (we're not applying for AP)

12/22/11 Received letter for Biometrics

01/04/12 Biometrics in Phoenix

01/31/12 Received email update from USCIS saying that we've been scheduled for an interview

02/03/12 Got an email saying card production had been ordered for my EAD...now just to wait for the card...

02/09/12 Received email saying EAD card has been sent

02/13/12 Received EAD card :D :D :D :D

03/06/12 Interview in Phoenix :).....Approved :D

03/14/12 Received Green card in mail.....

....no more USCIS until December 2013

oh ok i did mine in Sydney and i brang a express post envelope. It got sent to me first then so i could look at it then i posted it to the embassy.Must be different for each state :dance:

my son got his passport today and the medical is now moved to weds he is going to his local doctor to get up to date with his shots and to pick up his records they are cutting it close back in melbourne !! im hoping all goes well on the 15th of nov on his interview and he has everything
