Need Help Please

Hey guys I just got this Tuyên thệ độc thân (Affidavit of Single Status), Giấy chứng nhận chưa kết hôn (Certificate of No-marriage Records), and Bản sao hộ chiếu (Passport)documents from the Secretary of State of Washington State. Is it ok if I just send this to the Vietnam Consulate in San Francisco or do I need to attach the Application for Marriage Registration document with those other three document too? Also does any one know the address to the Vietnam Consulate in San Francisco because I have been calling them from this website http://www.vietnamconsulate-sf.org/main/vi/lanh-su all morning but no one pick up so if any one here know the correct phone number + address to where I can send this documents after receiving it back from the Secretary of State that would be helpful

Also do I need to send those document to the Consulate General of Vietnam in San Francisco or can I send it to the Embassy of Vietnam in Washington, DC or do I have to send it to the SF Consulate?

for this two documents Application for Marriage Registration, Bio Info Sheet do I need to have them certified by the Notary Public and Secretary of State? Also what does this "The bilingual form provided by the Embassy is for your convenience." mean? Does it mean it not necessary or a must have?
