Yes you need a copy front and back of I-94 for AOS

The I-485 and also the I-765 if you are applying for EAD

Since this is AOS thread I would think that it wouldn't be required with I-765 as it states in instructions:

If you are filing Form I-765 under the( c )(9) category, Form I-94 is not required
