Timeline

What forms do we have to carry to the Medical examaination?Do I have to carry the DS 3025? I am asking because I have not recieved anything from the consulate other than the 1 page interview appointment letter. Please respond because I have my medical exam on Nov 4th.

By the way, Breach Candy Hospital is no longer on the Panel for Immigrant visa medical examination.

2011-07-19 I-130 Approved

2011-08-01 NVC Received

2011-09-19 Receive Instruction Package

2011-09-30 Case Completed at NVC

1. Interview letter.

2. Passport & a copy of Bio-graphic pages of your passport.

3. 4 passport photos.

4. Immunization and Medical history reports, if any - else you may be given 3 vaccinations.

USA

01/08/13 - Approved and GC is order for production on 1/8/14

09/12/13 - Case transferred to CSC. NOA2 received on 09/18/13

08/30/13 - Biometrics Done - No walk ins allowed at this LSC (received on 8/16/13).

08/05/13 - NOA1 (received on 08/10/13)

08/01/13 - Mailed I-751 (received on 8/2/13 - check cashed on 8/5/13)

12/28/11 - Received SSN (applied on 12/20/11, as we didn't get based on DS-230 options)
11/28/11 - Received Green Card (Expires on 10/30/13) - Welcome Letter on 11/17/11
10/30/11 - POE - Houston, TX

Chennai Consulate (40 days)
10/28/11 - Received Visa papers and Passport at VFS
10/25/11 - Interview Cleared Successfully (Spouse was not allowed in)

NVC: (90 days from NOA2 to Consulate)
08/31/11 - Case Completed (Interview 10/25/11) - Received at Chennai on 09/19/11
07/22/11 - NVC Case Number

USCIS: (92 days)

6/21/11 - NOA2 (NOA1 on 3/25/11) - took a month to get to NVC
3/21/11 - I-130 sent to USCIS Lockbox, Chicago.

So NavyK, the medical examination form is filed by the doctor? I do not have to carry the DS 3025. right?

Timeline

Mine was at Lilavati hospital. Reached there by 8:30 AM. Did the registration. Filled out a simple questionaire regarding medical conditions. Then I had to pay 1600 bucks. I was then sent for a blood test (VDRL) and a chest X-ray( Pregnant women should not get a Chest X-ray done. So girls if you are pregnant, let them know). This followed with an ophthalmology examination. Then a nurse measured my blood pressure, weight (was very happy that I had not gained much even though I am being fed like a pig by my mom because thats what mothers do) and height.

Then the physical examination was conducted. Was asked a few questions about past medical illnesses. I had carried my family doctor's letter which mentioned my immunization history and said that I was fit. I was given 1 vaccine.

Got the reports at 4 PM. Had to wait for a very long time to get reports so take something to read.

Glad that its done.

Good. It is best submit the medical report sealed envelope (not the vaccination report sealed envelope - this needed at POE in US) at VFS along with their courier fee 387 rupees. Please go to VFS, Mumbai with Interview letter, passport and a copy of the bio-graphic pages, 3 passport photos, the sealed medical report and courier fee (allowed only 10 business days before I guess).

Good. It is best submit the medical report sealed envelope (not the vaccination report sealed envelope - this needed at POE in US) at VFS along with their courier fee 387 rupees. Please go to VFS, Mumbai with Interview letter, passport and a copy of the bio-graphic pages, 3 passport photos, the sealed medical report and courier fee (allowed only 10 business days before I guess).

NavyK they gave me one sealed envelope containing medical reports and one BIG sealed envelope containing the Xray and a loose vaccination sheet, just a single paper. The vaccination report is not sealed. Should it be sealed?
