Hi everybody! I have a copy of my DS-3025 form that I was given at the clinic after I completed the medical examination in Ukraine. Im applying for Adjustment of Status now. I wonder if I can just send the USCIS the copy of DS-3025 that i was give (btw, it states "incomplete... eligible for waivers because the vaccinations are not medically appropriate". so, can I just send that form? or do I still need to go to the civil surgeon and ask him to transfer my records to I-693 form?

If yours says INCOMPLETE you have to go to a Civil Surgeon to have him/her transcribe your record to I-693. :)

Were all the necessary shots given or were you told more were required?

The doctor told me that all my paper work was fine. He put all the documents in an envelop, sealed it and gave it to me as well as the copy of the vaccinations. He didnt saying anything about if more vaccinations were needed.

Then I would just send a copy of the DS-3025. If that's not okay theyll RFE you. Then you can worry about it then.

Please read the entire post. She is eligble for waivers.

Send the package with the DS-3025 and a written request for waivers, this is simply a letter stating that the vaccinations are not age appropriate, per the medical exam, and that you request a waiver from being required to have them

There is a good chance you will be fine.

If not, they will send an RFE. If they do that, THEN go to a civil surgeon with the I-693.

In the meantime, call around and get prices from CS's and their lead time for appointments so you are prepared.

By the way, what the doctor told you means nothing if you get an RFE. If they send you an RFE you have to do what it says.

There is no point in spending a lot of money to avoid an RFE that may never come. Be prepared for it. An Rfe will not slow your case down any more than trying to make a CS appointment now and holding off on sending the package. If it goes through you are ahead of the game. If it doesn't...oh,well.
