My now wife got here on a K-1 Visa. We've started to put together the AOS forms. We're about to see a Civil Surgeon in our area (Northern California).

We have a copy of the DS-3025 which identifies the vaccinations she received during her medical. IAW the CDC guide on what the required vaccination

is for my wife's age (41), she was required to have Tdap, MMR, Vacirella, and Inluenza. She received all of them except for Vacirella because she had

chicken pox before.

So, my question is...does the Civil Surgeon then essentially certifies the I-693 by completing part 5 and my wife completes part 1? I am asking because

I've called several CS on the list close to our area and none of them seem to know what I am talking about. I just want to make sure we're not missing

anything.

We completed our K1 in 2009 and this is what we did for AOS.

The USCIS doctor had us fill out only section 1 of the 693. The doc reviewed her embassy shot record 3025. She needed the same shots as you do. We went to the county health dept. and got the shots. We took the receipts back to the doc. Our doctor did not have the shots in her office and would have to order them. She recommended we go to the county instead. The doctor then filled out the rest of the form (part 5 I believe) and SEALED the 693 in a envelope. We sent this envelope with our AOS package. Do not tamper with the envelope or you will need to have the doc redo it.

Our doctor seemed to know exactly what to do since they do quite a few new immigrant 693's. Sounds like your doctors do not perform many USCIS 693's and may be rusty in the procedures. Unfortunetly the documents must be done correctly or RFE.........

If she's not missing any shots I would just send the DS-3025 (no need for the transcription). If however you want the transcription done then yes, your wife fills in Part 1, the CS completes the vaccination table in Part 2, and then signs part 5. That's it.

I had the same story like yours,we tried to find CS who can understand my situation base on the cs list at uscis.SEEMS like no body cannot understand,one we found somewhere in foothill fontana we tried to explain to death about the 1-693,stil had a hard time to understand,and then we came back and i print out the 1-693 and shows to them i answer the part 1 information about myself,i showes them my medical that i took overseas and they kept insisting to redo my entire medical,THEN good thing that i brought with me the INSTRUCTION from USCIS WEBSITE stating that if i had medical overseas i dont have to redo medical as long as taken less than a year,i explain to them 4 or more times,lol FUNNY.,then i highlighted the specific instruction,1 info about me part 2,VACCINATION PART ONLY part 5 the sc CERTIFICATION...HAHAHA finally ive done this the instruction that i printed out helped me alot..ohh thanks God,seems like they dont want to understand me..hahaha.they dont even know the form 1-693???im like what your kidding me right?they are on the uscis list how come they dont know?the assistant doctor kept asking me WHO was my lawyer????so i go what does the lawyer invloved with this??/i dont have any lawyer i do it myself,ALONE,,ALONE...ALONE ALONE....LOL...ITS JUST BECAUSE""""""VISA JOURNEY HELPED ME A LOT!!!THANK YOU VISA JOURNEY,VANNESA AND TONY WERE AWSOME!!!THANK U GUYS.....YOUR SUCH A BLESSING THIS SITE..

@ Gary, Vanessa & Tony, and Ronluvme...thanks a lot for the excellent info/advise. I will just have to print out the I-693 and instructions.

I have a feeling that I will have some explaining to do to the CS. I WILL NOT let the CS perform another medical exam...LOL

my vaccination record from slec says my vaccination is inc., but when the CS looked at my record he didnt give me additional vaccines he said i already meet the requirement vaccines of the USCIS... good that i have tdap, VH, and mmr.... :yes:
