Your wife needs her own tax returns and her own I-864 even if you have a co-sponsor. Each tax return (hers and her fathers) must be complete, which might include more than just tax returns and W2s, but every person is different. You might have schedules or other forms that make your tax return complete.

You only need a photocopy of your marriage cert.

AOS for my husband
8/17/10: INTERVIEW DAY (day 123) APPROVED!!

ROC:
5/23/12: Sent out package
2/06/13: APPROVED!

14.2 copy of 1-194 front and back

Few things.

The address is wrong, it will probably still get there but better to be correct. Please see the AOS instructions for the correct mailing address. Also known as:

USPS Mailing:

USCIS

P.O. Box 805887

Chicago, IL 60680-4120

For courier/express delivery:

USCIS

Attn: FBAS

131 South Dearborn - 3rd Floor

Chicago, IL 60603-5517

The "petitioner" no longer exists. The USC is now the sponsor and you are the applicant. they only complete the I-864, they aren't "applying" as your letter indicates they are.

11. I-864 doesn't need to be notarized. You also need to include the last years tax return, but can also include the previous other 2 years. if sending tax returns you include W2's and schedules etc or you can send tax transcripts.

12. Evidence of bonafide relationship isn't sent with the application

13.4 You write your alien number on the back of the photo, not receipt number

14.2 It's the I-94, not I-194.

on re-reading your list is lacking a few things. Please read the AOS thread in my signature.

Notably, the USC MUST complete an I-864 and include a tax return for the most recent tax year, as well as writing the income amounts on the I-864 for the 2 years prior. If not required to file returns, must include a letter explaining why.
