I'm sure this has been asked before but I've been getting mixed answers and so I am hoping for some clarification.

We are preparing to send in our AOS package and I am not sure how to file my medical. I've read the VJ guide and I am confused with regards to who needs to file the I-693. I have seen a couple of people say they sent in the vaccination sheet alone. My medical was completed in June so I am well within the 12 months of validity. I think I see the section that more or less says "Complete" or "Incomplete" but the box that is checked off for me is neither - it is the one that states I don't have every vaccination on the sheet but I am eligible for blanket waivers (i.e. not medically appropriate vaccines for my age range) - I'll let y'all look at the boxes for yourselves rather than try to explain it.

So I guess where I am confused is do I only need to file an I-693 if the "incomplete" box is checked off? What have others done?

Mods, please don't move this to the AOS forum - I am specifically looking for feedback from Canadian citizens who have the same medical sheet as I do because, based on conversations with several lovely friends from other countries, my form looks a little different. I have attached a photocopy of my vaccination sheet from the Toronto medical. Thanks!

Mine was marked as incomplete as well. due to just having the booster shots for MMR and Tetnus. But thats only becuase you cannot have shots back to back in a certain amount of time. I still just sent in the sheet with the checkboxes and it was ok. I would not worry about it.

quick edit..mine was done at the TO office as well.

I can't remember what boxes I had checked, but I just sent in the sheet form the medical with a letter explaining that my under their guidelines, my medical is still valid and the sheet I included shows my vaccination record.

I had no problems!

NOA1 - 8/16/11

Biometrics - 9/14/11

Mine was marked incomplete. This was back when they required the HPV vaccination and I only had shot 1 of 3. I just sent in a copy of the vaccination worksheet with my AOS, and it wasn't an issue. I had an interview and it wasn't an issue then either.

I had my medical today and the woman giving the instructions said not to panic when your vaccination sheet says incomplete. They all say incomplete, it just means you don't have every single vaccine on that list, but that you do have the required vaccines.
