hey every body im new to this forum

i got few question with me

im felling ds-230 part one and part 2

in ds230 part one number 2 ( other name used or aliases if married woman,give maiden name)

im married to us green card holder

my question is

before i married my first name was " Hina" and i dont have last time before married

when i got married i change my name to "hina sakoor"

in ds-230 part 1 number 2 what should i write down there

1-none or n/a

2 hina

3 hina sakoor

4 sakoor

please help me in this matter ....

my other question is

ds-230 part 1 number 31a( person named in 14 and 29 who will accompany you to the united states now)????

i have a doughter and she is only 10 months old should i put her name too

my third question is ds 230 part 2

number 43a

have you ever applied for social security number (ssn)?

i never been in usa what should i check

and in this there is another question

do you want the social secutrity administration to assign you ssn and issue a card? you must answare yes to question 43b.to recevie a number and a card )

should i check yes or no

please help me in this matter ..........

Since you are married put your middle name when you were single put"sakoor"

have you ever applied for social security number (ssn)?..In that questions as what you said you have never been in US..So again put."NO"

but in this ds230 par one question 2(other names used or aliases if married woman give maiden name ) but as per my knowledge maiden name is the name the last name used by woman before marriege but i dont have any last or middel name except my first name " hina " so how i can use my first name as a maiden name

#2 Put Hina only because as you said you don't have middle name so just write Hina.
