Can anyone give me some information. I'm pretty confused on the DS 230 form. Whose telephone number and address do I write down, mine or my petitioner's?

The beneficiary's info is needed on this form. It should specifically ask for the petitioner's name, address and phone number in question 39.

So in 39, it should be the petitioner's email address as well?
