Hello everyone! I moved to the US from the UK at the end of July and we got married last month. :) Now we're getting everything together to file for AOS. Everything seems to be okay, but I do have a question about the I-693, Report of Medical Examination and Vaccination Record.

My medical with the Knightsbridge doctors went without a hitch, although it was noted on the vaccination documentation worksheet that I was missing a second MMR shot. I chose to have this done by my own doctor and was instructed that I would need to provide a letter from them to prove that it had been done.

Looking at the information online, it doesn't look like I need to have another medical, but I'm confused as to whether or not I have to file the I-693 and see a civil surgeon. It feels like we're repeating a lot of the same information (though there seems to be a lot of that with this paperwork anyway, I guess).

Any information would really be appreciated! We can't wait to finally get this paperwork sent off and I can start feeling more settled! :)

Yes you have to file the I-693 filled by a civil surgeon. You can find the civil surgeon nearest to you here:

Take your vaccination records with you and the civil surgeon will conduct another test and ask you to come back in 48 hours. Make sure you send the form as you received it, it should be sealed.

You only need the vaccination portion of the 693 fixed because of the missing shot. You do not need a full medical over again.

Bring the proof of the shot and the vaccination copy from your previous medical, call around till you find someone who will do JUST the vaccination part for you. Go in and have them fix it up for you.

Thanks so much for the information. I'm still a bit worried though, since I presume the results from my medical were in the envelope I handed over at POE (except for the x-ray disc, which I still have) all I have is the vaccination documentation worksheet and the letter from my doctor to say I'd been given the second MMR shot. The I-693 seems to be a very detailed form - will it just be a case of a quick trip to the civil surgeon? Since they won't have access to my records, I'm just wondering how they'd be able to give me the okay without more tests...

I'm beginning to wish I'd spent the extra money having my vaccinations updated at the medical...it might have saved us some extra hassle now!

All they do is fill out the vaccination section of the form. They copy the vaccinations from your sheet and your letter of your extra shot over to the 693. They seal up the form and you DO NOT open the envelope. They give you an extra copy for yourself. You put the sealed copy in the AOS packet. Thats it. The doctor didn't even see me I waited at the front desk, took a matter of moments for him to copy over the stuff. ( i ended up not needing the 693 in the end too )

Read the i-693 instructions. You will see a vaccination transcription is all that's needed. That's Part 1, Part 2 (vaccination table ONLY!) and Part 5. There are no tests. They look at your shots and then give you what you're missing, transcribe it into the vaccination table, sign off on it, give you a copy, seal the original in an envelope and you mail that envelope. No stress.

Does anyone know if there would be a problem with getting the vaccinations and having the civil surgeon sign the I693 after the I94 has expired a few weeks? We are not sure what was turned in for medical. We have copy of the exam and xrays for visa and all was ok on that, but no mention can be found of vaccinations which have been had but early in childhood with no record. We are having a difficult time finding a CS in our area. The only one here is out on medical leave. We got married before 3 months but this i693 is causing us much stress. Thanks in advance.
