is Maiden name same with what's written on birth certificate?

hello all,

what should my husband write on a box of his G 325 A form which states:

CURRENT HUSBAND of WIFE

FAMILY (for WIFE, give MAIDEN NAME) ---????

the question is, what should he write on here? his last name? or my name with his last name added on it?

what makes us confused is, he thought that i don't have maiden name while my understanding is.. maiden name is the same with the name written on birth certificate? pleas help. thanks in advance!

my maiden name is my full name at birth - the one on my birth certificate.

Maiden name is the last name you had at birth, which is whatever was written on your birth certificate at birth.

the problem is I wrote my *SUPPOSED TO BE MAIDEN name* as *MY MIDDLE NAME* also (written on MY G 325a) would it be a problem?

the story is I only have 2 words for my birth name, example:

my name: Luvy Luv

so i wrote as follows:

First name: Luvy

Middle Name: Luv

Family Name: Corr ( Corr is my husband's last name) well since we are officially married, people suggested me to have it and write it as my Family name. soooo, now we got confusion in what to write on my husband's G 325 A since I HAVE WRITTEN my Maiden name as my middle name.

i hope you guys understand what i mean here, sorry if it's confusing

other friend suggests me to not use my husband last name as my family name and change the form so we wont get RFE. it's still teh beginning and it's a;ready a

Your maiden name is/was the name given at birth - it has nothing to do with your husband's name.

Your maiden name is/was Luvy Luv (the name you had at birth). You had no middle name at birth.

Your current name is Luvy Luv Corr. Currently you DO have a middle name.

Did you change you name to this? Is is on all your paperwork like marriage certificate, passport, drivers licence? Or did you just change your LAST name to Corr and leave your middle name blank?

Personally I used my pre-marriage name on ALL my marriage and visa paperwork. I will officially change my name after immigration is all done.
