INDONESIA - G-325A form: Family Name

I am the petitioner in the USA and I am filling out my G-325A form.

I have a question where it says, "Current Husband or Wife Family Name (For wife, given maiden name)"

The situation is that my wife has only 2 names, a first and a middle name. Her family does not have a "family/last name."

I was also told by some people that I could add my last name as her new FAMILY NAME.

So in this box where my question is referring to, what should I put in there? Do I put my last name or do I put her "maiden name," which she does not have?

ya know in my situation on my wifes ktp it ahs 2 names Y_____ N__________ and on her PAssport it has 3 names Y__________ N__________ S_____________

so i am putting her 3 name version on the forms and in other names used i am putting the 2 name version , the two name version is the one on her KTP And our MArriage cert.
