After I-824 approval,

Police certificates are required to be submitted.

Could you please tell me where are we supposed to get these certificates from.

Depends on the country. In most countries, if you live there you can get one from a police station or the Central Police HQ, it may take a while. If you don;t live in the country anymore, check out the country's embassy cllose to you's website, they will have details.

And what is the procedure to get the medical certificates

You need to go to an accredited doctor/ clinic to have that done. When you get your packet from the embassy, it'll have a list of doctors and you can make an appointment. They will take a health history, check you had all required vaccination, and do a physical including blood draw and possibly x-ray.
