Okay thanks, and the to pay for the forms? check? cash? everything has to be paid in 1 shot? ... what will the interview be like?

someone also told me earlier about doing a medical exam? do i need to do this?

Read easily accessible guides about AOS first, it will give you a good idea about the process and answer most of your questions. And please pay attention to replies to your post, somebody gave you his valuable time to answer your question about medical exam so least you can do is read it.

Yes the medical exam is required. You must go in and see the doctor and if you have a list of vaccinations you've taken before, take a copy with you. Like it was mentioned before, each form has copy of instructions with it with absolutely everything you need to fill out and additional evidence you need to send in. These forms will take some time to fill out and you have a lot of reading to do. All the guides are available here and you can download all the forms online from here or from the USCIS website. I am going through the AOS process and filed everything but it took me a while to read everything. The information in the instructions will tell you exactly what you need to do down to how to write out the check. Read it all and follow the instructions carefully. It can be done.

Yes. AOS cannot be completed without the medical exam. You must see a civil surgeon, USCIS has a list on their site.

Canadians often do not get any stamp or an I-94. The onus is on you to prove that you entered legally, however. Hopefully they swiped your passport.

I believe you, I meant I'm not sure about what you should put in that space..you might want to get a free consultation from an immigration lawyer or give your local office a call and just ask.
