NEED HELP ON I130 FORM

hi guys i am filling I130 for my wife and need help on 2 questions 13 and 14 on the form, i get confused.

-my first question on NUMBER 13 i am a Lawful Permanent Resident what should i put on this question? there are just check marks,should i leave it blank?

-my second question on NUMBER 14:Date and place of admission for adjustment to lawful permanent residence and class of admission,does it mean the date i entered the USA and from which port i entred?

Assuming you are referring to questions 13 and 14 about yourself (petitioner), then

13)Does not apply

14)you give date and place of admission printed on your green card (date is printed not sure if place is too, but you know this).. and also the class of admission (ex: CR-1, IR-5 whatever your situation is)

if you come here via the DV lottery then class is : CLASS DV1

date is on your green card

place is the first address you came to U.S

no , you should put the address you went to where you received your green card in th mail, that address you did put it the day they gave you a visa in the U.S consulat
