Customs form upon entering the U.S.

Does anyone know the customs form number that they give you on the plane before entering the U.S.? I want to pre-fill a copy out for my wife to have on the plane, then she can just copy the info over to the original, so that she won't have any problems.

IR-1/CR-1 VISA

06-13-2011 : Received DS-3032 / I-864 Bill
06-14-2011 : Pay I-864 Bill

06-16-2011 : Receive I-864 Package

06-20-2011 : Pay IV Bill
09-01-2011 : Case Completed at NVC
09-01-2011 : NVC Left
09-06-2011 : Consulate Received

10-21-2011 : Interview Date
10-21-2011 : Approved
10-25-2011 : Visa Recieved

Estimates/Stats : Your I-130 was approved in 106 days from your NOA1 date.

Lifting Conditions:

10-02-2013 : Filed I-751 Vermont Service Center

10-07-2013 : I-797 NOA1

10-19-2013 : Biometrics Date Received

11-04-2013 : Biometrics Complete

04-09-2014: IR-1 Approved

Timeline

She will only need the 6059B (link above in hikergirl post). An alien with an immigrant visa is not required to fill out an I-94.
