Adjustment of Status, PLEASE HELP !!!

I know this question has probaby been ask a thousand time before, But Please help me anyways. My wife has enter U.S on a K-1 visa. we got married within the 90 days as told. Now we need to adjust her status to a green card. She as had a medical exam and vaccine in Laos. When she enter the U.S. she had gave them a seal medical emvelope for entry. Now the question is, When filling out the I-485 do we need to complete the I-693 or a vaccine supplement?

You don't need to fill out the I-693 if you have a copy of the DS-3025 that you should have from the medical, unless the vaccinations needed weren't complete. That's what I was lead to believe, I had my interview for A.O.S. and I was good without a I-693. Here is the link, it is section 11: http://www.visajourney.com/content/k1k3aos

Dave is correct. If your vaccinations were up to date when you did the K1 then you don't need a vaccination transcription and need only send a copy of the DS-3025 (immunisation list) from your K1 medical. If you're missing shots then you need to have those shots and have those (and your shots from the DS-3025) transcribed into a section of the I-693 by a CS.
