Hi dear all

I apply for IR5 for my mother I already pay the IV fee today, now I am preparing DS-230 form and othe documents.

I use electronic processing. The consulate is in Guangzhou China.

Inside the DS-230 package, it should include:

DS-230 form,

civil documents(for beneficiary and petitioner).

photocopy of beneficiary's passport (picture page),

2 photocopies of benecifiary's passport style picture,

package 3 supplement sheet

Did I cover all documents above?

I have two questions:

1: When should I send OF-169 form? where should I send it to? OF-169 is a 9-pages form, should I just send the pages in Chinese or all pages.

2: Do I ask my mom to prepare a mailing address label (EMS label) from post office? should it be in Chinese or English?

OF-169 form is only used for:

K-1s

CR-1s, IR-1s that are doing consular intake on forms, usually called DCF, Direct Consular Filing.

To the best of my knowledge, this is not required for IR-5, or indeed any IR/CR visa type with document intake at NVC (as opposed to the Consulate).

Now come on, you know you want to get that silly GUZ IV P3 Supplemental Form done and outta the way, right? She needs to submit it with the DS-230 set.

Darnell, thank you very much for your reply.

So I don't need to send OF-169 to NVC, right?

In IV Instruction Packagae ("Package 3") section in the following link:

http://guangzhou.usembassy-china.org.cn/forms_and_packets.htm

it includes OF-169 form and say that NVC will not schedule interview unless NVC receives this form, but it doesn't mention that who need to fill in this form.

OF-169 form is only used for:

What's the difference of consular intake and document intake? My mom is in China now, she need to interview at Guangzhou, which one is she belong to?

consular intake of ds-230 - ds-230 is mailed directly to GUZ IV unit.

NVC intake of DS-230 - DS-230 is sent directly to the National Visa Center, somewhere buried in the trees of Vermont.

The OF-169 goes with packet 3 of the IV packet, the Visa Application set, for Direct Consular Filing (which you are NOT DOING)

Packet 3 intake, in your particular situation, is handled, instead, at the National Visa Center (NVC).

Darnell, thanks for your explaination, so I don't need to worry about OF-169 form now.

I have another question that when I send DS-230 package set, I need to scan passport style photo to pdf file and send it to NVC with DS-230 form.

I have passport picture digital file (2' x 2'), when scan to pdf, is there size requirement? I open it in paint program, then print it to PDF, but the picture is larger than 2' x 2', is it OK?

get irfanview, soonish. open yer file in it, print to pdf using the cutepdf print driver.

Hi, dear all,

I got an email from NVC today attached a CHECKLIST LETTER Response file. It ask me to send DS-230 package to NVC, it also say

"Please include a copy of this letter with your correspondence and write your case number clearly (exactly

as it appears below) on the upper right-hand corner of all documents."

So when I send DS-230 set to NVC, do I need to include this checklist cover page file or the cover page from where I pay the IV fee?

Also can I only put the case number on the righ-hand corner of the first page for each document or on all pages for each files?

If all it is asking for is the DS-230, then this is the 'classic false RFE' referenced in the RFE topic here in the IR-1/CR-1 section.

IMO, just include the coverpage from the web portal on the DS-230 e-submittal.

Some ppl use the first page, some ppl put it on all pages - but if yer doing EP, IMO the first page of something is sufficient.

I missed a bit - are you using EP for this IR-5, or no ?
