For the vaccination requirement I went to Concentra Care Clinic and they were very quick and knew just what to do. They also only charged me $30 which is not the worst I've heard of, so I wanted to share their info in case anyone is having a hard time finding a civil surgeon that will only transcribe the vaccine record to the I-693.
