DS-3025 vaccination documentation worksheet

This question seems to come up a lot but I haven't found a definitive answer. I am a K-1 visa applicant, who has had my medical examination in my home country and received the DS-3025 vaccination documentation worksheet from my doctor. I have not yet had my visa interview. The worksheet was marked "vaccine history incomplete - applicant may be eligible for blanket waiver" as some of the vaccines were deemed "age inappropriate" and others were marked "insufficient time interval" (I am missing the second MMR dose which needs to be given 4-12 weeks after the first one that I received during the medical exam).

Does this mean that I am OK for the initial visa entry, but that once I am in the US and ready to apply for AOS that I will need to have the vaccinations completed by a civil surgeon and then submit the I-693 with the I-485?

In the interest of saving time and money once I am in the US, would it be better to try to have the vaccinations completed while I am still in my home country, and then bring the proof of the final vaccinations with me? I read somewhere about a civil surgeon in Austin, TX that will let you send everything in and they will send the DS-3025 form back for only $10. Will they accept vaccination records from another country?

Is it possible to get the vaccinations completed after my interview but before I leave for the US, get a new DS-3025 from the panel physician that is marked "complete" and then bring that form for the AOS, or do I have to use the one from my original medical exam?

I read somewhere that some civil surgeons will just do the vaccinations part of the I-693 (anyone have any idea how much this would cost?). I also read that some will make you do the entire medical examination again (and charge you for it), which I would like to avoid.

Lanie if you have to complete the MMR then have it done in OZ and then take that with you to the US. It depends on what state you will be living in here in the US. I got my vaccination supplement transcribed over to the 693 for AOS for $50. No you don't have to have another medical at all as our meds are valid for 12 months. Just make sure you can get a copy of medical as sometimes when AOS is processing USCIS can have a tendency to lose medicals which they did with mine and wanted me to have TB and Syphllis test again. I was lucky I could get Sydney to send a copy to me by email and I found the results to the 2 tests in my x ray envelope. And no you wont need letter of intent again just take to the interview what is on your list to be taken. Good luck

You will probably need the second MMR dose come AOS time. You can have it before you leave but you would need to take the proof of the shot and the DS-3025 to a CS to have it transcribed.

The CS you're referring to that would accept transcriptions via post or whatever was doing it illegally. You MUST see the CS in person. You can see the CS as soon as you arrive. Dont need to be married. You can start (or your fiance can start) finding a reasonable CS in the area so when you arrive it'll be simple.
