Hi folks!!

I just received my Packet 3 from the Montreal Consulate office yesterday!!! Woo-hoo! :D I'm anxious to get the DS-230 form back to them ASAP. However, I'm unable to complete it confidently until I receive confirmation regarding these items:

1. The Montreal Consulate office provided me with a DS-230 form, and it's 2 pages in total (1 page back-to-back). However, when I opened up the pdf version provided on this site, that version has 4 pages (2 pages back-to-back). Could it be that Montreal forgot to attach the rest of it? For those of you who were sent packages from the Montreal location too, do you remember how many pages your form had?

2. Since Montreal sent me a copy of the form with the letter "K" already printed on the top left box of the DS-230 form, should I submit that same form, or is it okay to fill out the pdf version instead and manually write the "K" myself?

3. Question #25 of this form asks to list ALL previous visits to the US, and they only provide 3 lines. I've visited my fiance and other family in the US so many times that it might even take me a while to figure out all of the dates. Am I supposed to actually list ALL of my past visits, and if so, starting from what date? The date we started dating or since my first trip there when I was 7 years old? hehe.. :)

Any information would be greatly appreciated!! Thanks in advance!

For Question 25 put something to the affect of:

Multiple Visits to the USA since age 7.

Montreal understands that Cdn and US are very close and that alot of people doing shopping and site seeing especially if you live close to the border.

I put that and it worked for me.

I would use there forms.

Not sure about the DS 230, as I am a K3.

The consulate sent you only Part 1 of DS-230, used by most if not all consulates to initiate a K-visa application. Most consulates do not use Part 2 for the K visas (the things on it are covered on the I-485 application to adjust status), and those that do use Part 2 often have it filled out as part of the interview.

One last question...when sending the DS-230 form and checklist back to Montreal, where should I provide my name and case number? Can I just write it write on the top of the forms/documents? Or just on a separate piece of paper or the envelope?

The consulate sent you only Part 1 of DS-230, used by most if not all consulates to initiate a K-visa application. When I sent mine in, I attached another paper with my info on it (name, case number, etc) and it seemed to work because they didn't send it back to me. :lol:

When I sent mine in, I attached another paper with my info on it (name, case number, etc) and it seemed to work because they didn't send it back to me. :lol:
