In the K-1 visa guide there is the following tip:

"NOTE: Tip Regarding Vaccination Records

The K-1/2 beneficiary will be required by the embassy to have a "medical" done prior to the visa interview. Part of this interview will require that previous vaccinations are documented and potentially additional required vaccinations performed. Once this is complete the doctor will note this on a DS-3025 form that will be sent to the embassy. During the medical appointment request an official copy of the vaccination records from the physician (or a copy of the completed DS-3025 if they will give it to you) as this will be helpful to have on hand in the future (specifically it can be helpful when filing for Adjustment of Status if your DS-3025 form is not available)."

When my fiancee, Jenni, had her medical she asked for a copy of the DS-3025, but what they ended up giving her was some Vaccinations form that said "For Use in Canada Only" or something similar. Can this be used for the AOS somehow so she doesn't need to get her shots again? Or does she need to insist on getting a copy of the DS-3025?

yes she can use this for the AOS process.

edit to add, my vaccination sheet from the medical said that as well. NO problem with AOS using only that sheet.

Did you need to fill out an I-693? The instructions are confusing. It says if you were admitted to the U.S. as a k-1 fiancee you don't need a medical if within one year of the overseas medical, but you still have to show proof of vaccination requirements. How do we show proof? Just send a copy of that vaccination sheet or do we need to get a Civil Surgeon to fill out a vaccination chart and get their certification on the I-693?

No 693. Just the vaccination sheet from the medical in Canada.

Hi ...

I think the medical package (that is sealed) contains the physician's completed 3025 and that is on your file, hopefully, when you submit your 485. I think the advice above is suggesting that you should try to obtain another copy from the physician just so that if for some reason they misplace it you have your own copy. I'll try to find where I read this, and let you know.

No, at the medical they give you a copy of the vaccination sheet. You keep this and send a copy of it with your AOS.
