does the vaccination form need to be filled out by a US surgeon or can my doctor fill it out in my homecountry? thanks

Did you have the medical in your home country for the k1 visa? If you did and they filled out the vaccination form and marked it as complete then you do not need to send anything with your AOS. From what I understand nobody usually gets a complete one which means yes your will need to go to an approved civil surgeon and have them fill out the vaccination form of the medical ONLY.

The Medical Form should be filled by USCIS approved civil surgeon.

You can check online for USCIS approved civil surgeon by entering your area code.

https://egov.uscis.gov/crisgwi/go?action=offices.type&OfficeLocator.office_type=CIV

If you don't have the record of vaccination, don't worry. The surgeon will send the blood sample to lab to confirm vaccination.(There is certain criteria to identify the antibodies in the blood for different vaccine) If any vaccine is low in your blood he will administer that vaccine to you.(The vaccine price cost seperately/he charge seperately).

He will do a TB PPD test, if it is negetive its ok. If it is positive (Certain Asian and Europian countries give TB vaccine to kids which shows positive all the time)then you will go for Chest X-Ray.

if my parents will send me the records over then the surgeon will just transfer the records on my medical form? sorry if i'm asking too many questions, just trying to get everything done the right way :)
