Many people said that If my finace had an exam done in her country as part of the requirement to get K 1 visa and she has a copy of DS-3025, we don't have to send i-693 as part of the adjustment of status packet, am I correct?

I read this in the guide:

If you are a K-1, K-2, K-3 or K-4 visa holder the I-693 instructions state that you are not required to have another medical examination as long as your Form I-485 is filed within one year of your overseas medical examination. Regardless of if you are exempt from a medical exam, you are still required to complete certain portions of the I-693 and have a Civil Surgeon certify the form (and seal it in an evelope). Specifically, you will complete Part 1 (Information about you) of the I-693 and provide both the form and your DS-3025 (if you have it, proving your vaccination history) and any proof of required vaccinations that were completed prior to entering the US. If you do not have a copy of your DS-3025 you will need to provide sufficient evidence of your vaccination history to the Civil Surgeon (talk to the civil surgeon's office to discuss what vaccination records they accept as proof). If you do not have proof of your vaccination history the Civil Surgeon may insist on re-administering all the required vaccinations prior to completing the I-693. Once the Civil Surgeon has has verified that all required vaccinations have been performed, they will complete form I-693 Part 2 (the vaccination section) and Part 5 (the Civil Surgeon's Certification). They will then seal the form in an envelope and return it to you.

The part in bold makes me a little bit confused now, so we only need to fill out the part about her and have it signed by the surgeon without any medical exams? I'm not sure what's the point of this.

Is she missing any shots? If she's not then she doesn't need the I-693 transcription.

If she's not sure, she should see a CS and have her shots transcribed into the I-693. Read the i-693 instructions to see that that's only part 1, the vaccination table ONLY of Part 2, and the signature section (Part 5 I believe)

Yes she had all vaccinations done. In the form instructions it says we don't have to send i-693 if we have completed DS3025

Hi there,

When we were filing our AOS packet I stressed about this I-693 profusely as to what should be done. Since I had received a medical in Canada and it was still within the guidelines (under a year) I only submitted the vaccination worksheet with my packet which is similar to a DS-3025 as in Canada they don't call it the DS-3025 it is only called vaccination worksheet. I was never asked for it at the interview either and I received my 2 year Green card no problem. Since it costs money to get the I-693 filled out by a civil surgeon, it is up to you whether you pay more money and get it done but from my experience many people that I heard from on Visa Journey at the time when I was filing AOS advised me that they just submitted the DS-3025 and were fine. If the USCIS needs further information then they will ask for it so why pay money when the DS-3025 might be all they need. Obviously, it is your decision which way you decide to go but just wanted to let you know my experience. Hope this helps.

I can't really see your attachment, it is kinda blurry but it basically listed all the vaccinations and the dates of vaccinations on it. The doctor signed it at the bottom and said that I was eligible for a waiver or something like that. Hope this helps.

If the beneficiary's vaccinations were COMPLETED as part of the visa process, IT IS NOT NECESSARY TO INCLUDE FORM I-693 IN THE AOS SUBMISSION. All your medical information is enclosed in the brown envelope which was handed in at the POE. The envelope is then forwarded on to USCIS where it will be consolidated with the I-485 submission, once received. If you received a copy of the Vaccination Documentation Worksheet (Form DS-3025) from your Panel Physician, you may include a copy of that form in your AOS submission in place of Form I-693.

Note the words "were COMPLETED as part of the visa process" if you have the vaccination form DS-3025 and it is marked completed then you do not need I-693. There is a check box at the bottom of the form to indicate they are complete.

My wife's was not marked complete so she had to have just the vaccination worksheet filled out at a CS.

My wife's was not marked complete so she had to have just the vaccination worksheet filled out at a CS.

It is almost IMPOSSIBLE to have it ticked "completed" what they mean is your shots were supposed to be complete for your age range/health and even if they are, it will STILL be ticked "not completed" because the only way to have it ticked completed is to have every single shot on the list complete (if the panel physician follows the instructions). Mine were so I did NOT need a transcription, I got one anyway for other reasons.
