Why don't you add this to the "Civil Surgeons who don't rip you off" thread so it can be found more easily later? $5 might be a new all-time record!

AOS for my husband
8/17/10: INTERVIEW DAY (day 123) APPROVED!!

ROC:
5/23/12: Sent out package
2/06/13: APPROVED!

I think I have a ''better deal "'. The Civil Surgeon in Pensacola Florida told me he transcribed the vaccinations for FREE. Unfortunately we had to do a skin test and blood test and the transcription --so we had to pay .

Why did you need a skin and blood test? A K-1 adjusting status does not need new medical tests. It sounds like they would not do JUST the vaccination transcription, so your post in the civil surgeons that don't rip you off might be a bit misleading if the doctor insists on giving these extra tests.

It was indeed FREE to just transcribe the vaccination records . If your Vaccination record DS-3025 is marked ''Incomplete'' --you have to see a civil surgeon to do a skin test and a RPR blood test and a brief checkup . Certain countries does that like the philippines . They advised you to do it since many persons received RFE for not doing it .

If my wife record was marked ''complete'' --the transcription would have been FREE.

Only if your DS-3025 is marked "incomplete" is a I-693 required to be completed. If it is marked as such (incomplete) you are required to complete certain portions of the I-693 and have a Civil Surgeon certify the form (and seal it in an evelope).

My wife had to do a skin test and a RPR (a blood test ) in addition to having the vaccinations being transcribed . I am playing it safe . It is better than to receive a RFE later as the surgeon pointed out .

Yes, if your vaccinations are incomplete, you are required to see a CS and get certain portions of the I-693 filled out. Those portions are: Part 1-info about you, Part 2-vaccinations section only, and Part 5-CS signs off on the form. That is it. Another TB test, other blood tests and/or a brief medical checkup are not required. If your medical was within 1 year of filing for AOS, you do not need another medical exam or testing.

From the I-485 instructions: http://www.uscis.gov/files/form/i-485instr.pdf

Page 4- #8 Fiancé(e)s

If you are a K-1 fiancé(e) or K-2 dependent who had a medical examination within the past year as required for the nonimmigrant fiancé(e) visa, you only need to submit a vaccination supplement, not the entire medical report. You may include the vaccination supplement with your Form I-485.

From the I-693 instructions: http://www.uscis.gov/files/form/i-693instr.pdf

Page4- Section III Frequently Asked Questions

#2 What if I am a K nonimmigrant visa holder and already had a medical exam overseas?

If you were admitted as a:

A. K-1 fiancé(e) or a K-2 child of a K-1 fiancé(e); or

B. as a K-3 spouse of a U.S. citizen or a K-4 child of a K-3 spouse of a U.S. citizen; and

C. You received a medical examination prior to admission, then:

  • a. You are not required to have another medical examination as long as your Form I-485, Application to Register Permanent Residence or Adjust Status, is filed within one year of your overseas medical examination, and the medical examination did not reveal a Class A medical condition, or if you did have a Class A medical condition, you received a waiver of inadmissibility and have complied with the terms and conditions on the waiver.
  • b. Even if a new medical examination is not required, you still must show proof that you complied with the vaccination requirements. If the vaccination record (DS 3025) was not properly completed and included as part of the original, overseas medical examination report, you will have to have the vaccination report completed by a designated civil surgeon. In this case, you are required to submit Part 1, Information About You, Part 2, the vaccination chart and Part 5, the Civil Surgeon's Certification, of Form I-693.

No. A skin test is ONLY required if you are undergoing a new medical.

Most DS-3025's will be marked incomplete because it's basically impossible to have EVERY single shot on the sheet and also not a requirement. If your shots are up to date for your age range then you need only send the DS-3025 with your AOS packet. This is evidenced by many many people doing this without RFE. I personally just did the I-693 transcription because at the time of my filing RFE's were rampant for missing medicals and I was hoping to stop that by showing I ONLY needed a transcription. I didn't need any extra shots. I did not have a skin test or xray or anything else. Just a transcription. No RFE's and no interview.

Not exactly. If your SHOTS are incomplete you need to see a CS and get those shots and get those, and the other shots transcribed in the manner you explained. However if your shots are up-to-date but your sheet says incomplete (which is normal as several shots are not age appropriate for most people)

I explained that to the nurse and she was transcribing the vaccinaqtions for free , but advised that many persons received RFE from certain countries if they did not do the skin test and 1 more blood test . I was just playing it safe . Thanks for the reply . I hope all goes well . I am filing soon .

She lied. Just FYI. They tried telling me the same thing but it's a typical scam to get more money. I promised to not come back if I was RFE'd for a missing TB test... I was never RFE'd because, as I said, it's NOT required.
